Remedy has implemented a secure cloud-based health-and-medical record system.  Privacy Policy

In order to use this new feature you will need a valid email address and a personal identification number (PIN#). To obtain your PIN#, call us at 630.324.6019.

Once you've obtained your PIN#, you will have the ability to complete your initial patient intake and health history information online from the privacy and comfort of your own home, office, or mobile device. In addition, once registered, you will have the ability to conveniently schedule future appointments online, and update changes to your health history.
